Incident investigation

Western Sydney University is required to investigate incidents and develop corrective actions to prevent reoccurrence.

Investigations must occur as soon as reasonably practical after the incident to ensure accurate information is obtained.

Supervisors and managers are required to investigate all incidents within areas under their control. Where a notifiable incident has occurred, the WHS unit will appoint an incident investigator.

The University has adopted a root cause approach, based off the ICAM model, to incident investigation.

The root cause model allows the investigator to consider contributing factors and underlying causes which may have contributed to the incidents occurrence.

The WHS unit is developing a an Incident Investigation Guideline to assist supervisors and managers complete incident investigations. In the interim, for further information on how to complete an incident investigation please contact the WHS unit.
