Organising a COVID Safe event or activity

Due to updates made to the Public Health Order there is NO LONGER A REQUIREMENT TO FILL IN THIS FORM. The following are recommendations:

1. Wearing Masks
2. 1.5 distance (this is approximately 75% capacity)
3. Having hand sanitiser.

Only those who are using public transport or attending airports, healthcare/ age care workers and students are required to wear masks.

Please remember we are still in a pandemic and there is still a requirement to report to COVID Positive staff to notify the University via the COVID Positive Report Form and students to contact whs@westernsydney.edu.au

It is important that you complete a risk assessment to ensure all other WHS&W hazards are controlled.
