To request supplies for the standard First Aid kits, please complete the following form.
Please note: you must be a designated First Aid Officer to order supplies.
In the drop-down menus, select the individual items that need replenishing and the quantity of each item that is required. Please note that each item has a prescribed quantity that cannot be exceeded. One form should be submitted for each First Aid kit.
The requested supplies will be dispatched through the internal mail to the campus designated below.
